As of January 1, 2021, Pinellas County's vision plan moved to EyeMed from Davis Vision. The plan is similar, but there is a different provider network which includes LensCrafters (but not Visionworks).

While most vision insurance policies cover basic eye exams and treatment, they do not cover exams for more significant eye diseases. Davis Vision provides coverage for other types of eye testing, which can be included during your routine eye exam, like pressure tests for glaucoma or slit lamp tests to examine your retinal health. When you visit an eye doctor, your vision copay is a fixed amount you pay for services. So when you get an eye exam, you pay a copay and your plan covers the rest. Your vision plan identifies what your copay is for vision services. Why is a copay necessary? Vision Insurance at Davis Vision. Rather than being an extension of a dental plan or another product in a company’s line of services, Davis Vision’s main focus is on providing quality eye care services. It is therefore no surprise that Davis Vision offers one of the most comprehensive lists of eye care services, and is a solid place to shop.

Pinellas County provides vision coverage to its retirees through EyeMed. You pay no additional premium for vision benefits. They are included in your health insurance deduction.

  • Visionworks does not participate in the EyeMed plan.

ID Card

  • EyeMed provides ID cards to members when you enroll.
  • If you lose your ID card, call EyeMed at (866) 939-3633 or login to EyeMed, select Account, and Print My ID Card.
  • Please note that you don't need an ID card to use your vision benefits, just your name and birth date.

Network Benefits

When you visit an in-network eye doctor, you don’t need to submit a claim. EyeMed handles the paperwork.

Basic Eye Examination:

  • $10 copay per calendar year for vision exam
Eyeglass Lenses:
  • $20 copay per calendar year for clear plastic lenses in single, bifocal, trifocal or lenticular prescription

    • Eyeglass frame allowance: $130 allowance

Out-of-Network Benefits

  • Refer to the EyeMed Summary for out-of-network reimbursement benefits.
  • Use the EyeMed Out-of-Network Claims Form if you visit an out-of-network provider for routine eye care, and submit the form to EyeMed for reimbursement.

LASIK or PRK

  • IMPORTANT: You must call (800) 988-4221 for EyeMed confirmation before scheduling laser vision correction service.
  • Find an eye surgeon in the U.S. Laser Network.
  • 15% off retail or 5% off promotional price less $1,125 allowance

Frequently Asked Questions

Do I go to an EyeMed provider if I need eye drops, have an eye infection or have injured my eyes?
No. The vision care benefit covers routine vision care only such as routine eye examinations, corrective eyewear and LASIK/PRK. Services related to medical or surgical treatment of eye disease or injury are not covered by EyeMed. Contact a UnitedHealthcare medical provider for services related to illness or injuries to the eyes.

Where can I go for a vision exam?

See EyeMed's Find an Eye Doctor and select Insight for the network or call (866) 939-3633.

What if my current provider is not in the EyeMed network?

EyeMed contracts with an extensive network of providers in the marketplace including LensCrafters. If your current provider is not in their network, you may select a new provider or use the out-of-network reimbursement option.

How often may I have a vision exam?
You may receive a basic comprehensive routine vision exam once per calendar year for a $10 copay.

How often may I purchase eyewear?

Eyeglass lenses and frames may be purchased once per calendar year.

Can I get vision services if I am out of town?
Yes. EyeMed is a nationwide program. To locate a provider, see EyeMed's Find an Eye Doctor and select Insight for the network or call (866) 939-3633.


Will I have to pay more than a $20 copay when I purchase eyeglasses?
Clear plastic lenses are covered under the $20 copay including single vision, bifocal, trifocal or lenticular lenses. Eyeglass frames allowance is $130. Please see the EyeMed Summary for more information.


Will my contact fitting fee be covered?
Yes. Contact fitting and follow-up care is free of charge.


May I use my vision benefit to purchase both eyeglasses and contacts?
No. You may purchase either eyeglasses or contacts.

Insurance

Coverage under the plan includes oversize lenses, blended invisible bifocals, certain fashion frames and sun tinting of plastic lenses, gradient tints, glass gray #3 prescription sunglasses, and initial lenses following cataract surgery. Materials carry a one-year warranty against breakage. Discounts are available for nonprescription and additional prescription glasses.

The plan covers the following services once each calendar year.

  • Up to $125 on retail frame allowances
  • An additional discount of 20 percent on any overage for frames
  • An additional discount of 15 percent on any overage for contact lenses
  • Access to the Scratch Protection Plan:
    • single vision, $20
    • multifocal lenses, $40
  • Access to the Premium Contact Lens Formulary
  • Fully covered medically necessary lenses (In-Network providers).

Eye Exam

Davis Vision provides a comprehensive eye examination, including a review of your case history, health status of the visual system, refractive status evaluation, binocular function, diagnosis, and treatment. The eye exam is covered in full with a Davis Vision participating provider. The maximum reimbursement with a non-participating provider is $30.

Please note: Your health plan may cover one eye exam annually. Contact your health plan directly to ask if eye exams are covered.

Eyeglasses

Davis Vision has developed its own laboratories from which all plan lenses are made. Your participating provider will automatically send your prescription to the Davis laboratory where your lenses will be made.

Lenses

Lenses are covered in full with a Davis Vision provider. The maximum reimbursement with a non-participating provider is:

  • Single-vision lenses, $25
  • Bifocal lenses, $35
  • Trifocal lenses, $45

Eyeglass Frames

You can choose from Davis Vision's Fashion, Designer and Premier Collections. There are approximately 300 frames to select from, and these collections can be seen in any participating provider office. There will be an additional cost for some frames. There is a one-year warranty against breakage on all in-network plan-provided eyeglasses.

Fashion, designer, and premier collections are covered in full with a Davis Vision provider or receive a $50 credit toward a network provider's own frame. The maximum reimbursement with a non-participating provider is $30.

Additional Options

The following options are covered in full with a Davis Vision provider and are not covered if you go to a non-participating provider:

Additional Plan Enhancements

If your prescription requires additional enhancements, a copay will be added; however, the costs are generally at wholesale prices when ordered through a Davis Vision provider.

  • Polycarbonate lenses, $30
  • Glare-resistant treatment, $33
  • Polaroid lenses, $60
  • High index lenses, $55
  • Premium Glare-resistant treatment (anti-reflective coating), $48

*Progressive Addition Multifocals can be worn by most people, but not all. Conventional bifocals will be provided to those unable to adapt to Progressive Addition lenses; however, the copay will not be refunded.

Contact Lenses in lieu of Eyeglasses

Some contact lenses in lieu of eyeglasses are also covered in full with a Davis Vision provider. Contact Davis Vision to verify that your brand of contacts is covered before you enroll in the plan. The maximum reimbursement with a non-participating provider is $75.

  • Standard daily wear soft lens users will receive one pair of lenses.
  • Disposable contact lens wearers will receive four multi-packs of lenses.
  • Planned replacement contact lens wearers receive two multi-packs of lenses.

Non-plan Contact Lenses

A $105 retail credit will be applied toward contact lenses from the Davis Vision provider's own supply. Non-plan contact lenses are not covered if you go to a non-participating provider.

Please note: Contact lenses can be worn by most people. Once the contact lens option is selected and the lenses are fitted, they may not be exchanged for eyeglasses.

Laser Vision Correction Services

Davis Vision Insurance Company

Davis Vision provides you and your eligible dependents with the opportunity to receive Laser Vision Correction Services at discounts of up to 25% off a participating provider's normal charges, or 5% off any advertised special (please note that some providers have flat fees equivalent to these discounts). Buy a Voucher Program

You can purchase additional pairs of eyeglasses or contact lenses directly from Davis Vision. To speak with a Davis Vision representative, call 1-800-999-5431. You will hear a prompt to 'Enter the member's ID number.' Your Davis Vision member ID number is your UMID or your Social Security Number. If you don't know your member ID number, or wish to bypass the menu options, simply do nothing. The prompt will repeat two more times, and when you don't enter a member ID number after the third prompt your call will be automatically transferred to a Davis Vision representative.

Ask to purchase a 'Buy-A-Voucher.' A Member Service Representative will verify your eligibility and help you with your purchases. Available services and costs are listed below.

Material prices do not include an eye examination and are applicable to plan frames and lenses.

All prices include a $9 administration fee. The administration fee applies only once for any service or combination of services. These prices are subject to change.
